We had an issue just the other day which was a PERFECT example of one way this issue can show up:
Player goes to join the game like he always does. Gets a 'player doesn't exist' message. After messing around for awhile, he realizes that when starting the game up, the email part of the login has focus. If you hit a key as the game is starting, you can accidentally add unwanted characters right there at the start of the email, leading to the Auth database getting confused. I personally always do this by hitting my TS key while the game is loading. This isn't always the cause, but it's a good place to start looking...
